COMMUNITY HEALTH GRANT: Now Open!
Mount Baker Foundation will invest in community-led initiatives that advance health equity and improve long-term health outcomes through diabetes and kidney disease prevention.
We believe that health is shaped not only by medical care, but also by the conditions in which people live, work, learn, and play. For this reason, we will prioritize funding to organizations working on upstream strategies that address the social determinants of health and their relation to diabetes and kidney disease, such as access to nutritious food, safe environments for physical activity, culturally relevant health education, and supportive community networks.
Funding Priorities
We seek proposals from organizations that are committed to reducing health inequities and serve communities disproportionately impacted by chronic diseases and barriers to healthcare access, including:
Communities of color
LGBTQ+ communities
Rural communities
Disabilities communities
Low-income and uninsured populations
Immigrant and Migrant Communities
Other groups historically excluded from quality health care

Examples of Projects We Would Fund
A rural community center is creating a culturally tailored nutrition program that partners with local farmers to increase access to fresh produce for families at risk of diabetes.
A tribal health program is launching kidney health screenings paired with traditional food education to reduce risk factors in Indigenous communities.
A community health center is training community members as peer health educators who support neighbors in attending medical appointments, understanding treatment plans, and accessing preventive care services.
